Once completed, the LivingMatrix automatically maps your information into the Functional Medicine Timeline and the Matrix.
When you complete the Patient Health Update forms online (through LivingMatrix, not the one on our website) before your follow-up visits, this information is automatically edited and added to your health narrative, which you can access online.

- Please sign up for LivingMatrix by notifying our office that you want to use the LivingMatrix option to complete your health-related questionnaires.
- After we sign you up, you will receive an email from “support@livingmatrix.com,” which comes from our LivingMatrix Application. Please check your “spam” file if this email does not show up in your usual inbox.
- In this email you will have a customized link to your initial intake paperwork. Please click the link and choose your personal password. Click Login to begin entering all patient information.
- Please make sure to fill out all information thoroughly as it is integral to our “root-cause” analysis approach.
